Email was designed for text. Simple ASCII text. Images, and other Binary files, are add on hacks. What is needed is a new overall system. One that can deal with binary files, formating, video(yea i know binary files). It should work both in real time communication(instant messaging, to voice, to video chat depending on bandwidth and desire) But it should also be able to hold messages for delivery later(like regular email).

The biggest hurdle though is bandwidth. Not everyone has broadband, so the low speed options(instant message, text based email) would need to be compact. But the ability to have both real time and delayed recieved video messages should be an intergral part.
